1. To get an access to preparatory distance courses in biology and chemistry, you need to fill out this questionnaire with user's contact details, then to make payment by entering the bank card details on the automatically opened page of the LLC Electronic payment company "ASSIST" website.

2. Access to the remote preparatory courses is provided no later than within two days from the moment of ordering the service by sending a login and password to the e-mail address indicated in this questionnaire.

3. After providing access to the Customer by transferring data for authorization (login and password), no return of funds from the Contractor to the Customer is assumed.
