On October 24, 2024, the head of the Department of Evidence-Based Medicine and Clinical Diagnostics of the Faculty of Advanced Training and Preparatory Studies, Doctor of Medical Sciences, Professor I.V. Zhiltsov gave an open lecture to the 4th-year students of the Faculty of Medicine on the topic "Assessment of the clinical significance of diagnostic tests from the standpoint of evidence-based medicine".
During the lecture, the principles of organizing and conducting clinical trials of diagnostic tests in accordance with the requirements of evidence-based medicine were described, the characteristics of the clinical significance of diagnostic tests (diagnostic sensitivity and specificity, diagnostic effectiveness of the test, the prognostic significance of positive and negative test results, the likelihood ratios of positive and negative test results), methods for calculating them and principles of evaluation were described.
The concept of sensitive and specific tests, screening and confirmatory tests is given. From the standpoint of evidence-based medicine, the concepts of norm, pathology and "gray zone" were substantiated, as well as the sequence of application of diagnostic tests with different characteristics for establishing a diagnosis of diseases. The concept of operator-dependent diagnostic methods is described, their shortcomings and limits of applicability are characterized. During the lecture, the need for doctors to meaningfully prescribe diagnostic tests, the results of which should influence the tactics of diagnosis and treatment of patients, was especially emphasized.
